There are LOTS of traditional courses that teach clinical skills that can bring results. Teaching suturing on pig mandibles, on bananas or on plastic models with fake “gums.

Like I said, I've done countless hands-on courses.

So I like giving them and recommend them.

Live surgical courses will always be powerful part of your training.

But when it comes to learning how to suture, I’ve seen that dentists still don’t really get the “suturing thing” in a live course. Doctors get the essence of the surgery but the suturing is taught in a haphazard way. No step-by-step, not strategy, no tips. It’s almost like they expect to know how to suture going into the course. It’s almost like they take this skill for granted. 

Was that your experience as well?

Regardless...

I was thinking, “man, what happens to the dentists that take my courses afterwards, when coming back to their practices? Can they suture?? Did I do a good job teaching them??"
 

• Are they clear about creating stable knots?
• Will their flaps open after bone grafting?
• What happens when they suture a graft and it gets loose?
• What if they get overwhelmed - gasp! – and don’t perform surgery because of that??

I wondered...

And so - you know me - I got obsessed with the topic and started experimenting.

I knew that people love to solve quizzes, compete, get challenged and basically play. The instinct to play is engraved in all humans since we’re babies.

I thought to myself, “What if I can turn learning how to suture into a game?”. And this is when I thought about the idea of “GAMIFICATION” and the zPAD system was born!

If you know anything about me, I love testing new things in my teachings. I love experimenting and finding new and better ways to teach.

So I literally went into my garage and tried a lot of things. I used commercial silicon and create a device that will help me teach suturing and also be fun like playing a game.

I made models of clinical situations that require suturing, combined them and took a huge impression with buckets of silicon. Then I create a mold and poured in another silicon which created the first suturing pad.

I started testing PLAYING WITH IT with different type of suturing and IT WAS FUN!

It looked awful and smelled bad but I knew there is something there to explore.

What I didn’t know is that the one of the byproducts from these chemical reactions is CYANIDE! Since I didn’t want to be found dead from cyanide poisoning holding the suturing pad (now called the zPAD), I called one of my patients who does this for a living – Ian (I practice next to Hollywood so there are plenty of professionals like that here).

I gave him my models, prototypes and the concept and within 2 weeks we had the zPAD 1.0. Always call a pro to these thing. I highly recommend it.

I put it to the test immediately. I started practicing with the zPAD, suturing it while documenting every step.

Created the exercises teaching the suturing techniques. Techniques every dentist must know. Each exercise is a fund challenge. When teaching is gamified, it’s not intimidating and encourages you to move forward.

This is when the 9 LAWS OF SUTURING WERE BORN…

To simplify things, I focused on what’s critical to be excellent at suturing and the answer is:

Safety, materials and mechanics

Each topic has 3 rules (3X3=9 LAWS)

I wanted to teach dentist EXACTLY what I’m doing even the details of how I hold my needle and sutures, how I twist, How I lock and release, my hand movements etc.... So I filmed myself and created short tutorials. Of course a step-by-step manual was the natural next step (created over 200 illustrations of different  suturing techniques).

I took this massive topic and broke it down to the basic elements so every dentist at any level will be able to understand, follow and most important IMPLEMENT in surgery.

How? If you took something complex and break it down to the molecular level, every step and every building block is actually super simple. Wow!

What I found was amazing to me. A comprehensive, detailed and really simple suturing teaching was born.

Honestly, it took a long while, and a lot of mistakes, to figure out how to structure something like that.

Lots of educators theorize about the effectiveness of their teaching, how they lecture and deliver material, esthetics of the slides and final result and things like that. That’s all great but if nobody applies these teachings, is that successful education?

For me, I do my best create new teaching concepts and then I go and test them immediately. I'll try almost anything that is good for the doctors that I mentor.

I consider it a success only  if I’m able to I motivate a doctor to take action and succeed in their practice by themselves.

So the onus is on me 100%.

If I do a good job, you will be successful. I you can’t do it, I have to go back to the drawing board and find better way to teach, inspire and motivate you. Seems like a fair deal to me.

So back to the zPAD;  The moment of truth has arrived - Will it work?

Is it possible to teach dentists in a short period of time, like 7 days or even condensed into 1 day, all they need to know in order to master suturing?

Think about it:

What if a learning the art of suturing can simple, fun and quick and

completely different than what everyone else is doing?

Dentists have 2 main “surgery problems”: (1) Incision and flap design (2) suturing.

The beginning and the ending points of surgery which are super critical. So with suturing I decided to nail once and for all with the zPAD.

Honestly, I'm surprised dental educators haven’t tried to crack this one before me considering suturing is critical.

So here was the first LIVE try:  tested on 32 dentists and one non-dentist (a normal human being;-).

I started by mentoring them on the basic principles and the 9 LAWS OF SUTURING (safety, material and mechanics). Then I set them each up with a zPAD, basic suturing kit, video tutorials and I’m in the background.

It was like “Set it and forget it!”, B A A A M!!

The first time I did it, I couldn't BELIEVE how quiet the room was with dentists going through the different exercises, from one challenge to the other.

Total focus and concentration.  Having fun with something that was “difficult” and “impossible” before and now through gamification, they are getting better and really get it. Almost without exception: After a total of 7 hours their suturing understanding and skill has been transformed.

I looked at their completed zPADs…

I had to check all of them MULTIPLE TIMES. Honestly… some did it as good as me and some better than me.

How about the non-dentist in the group (the normal person)??

THE NEW TEACHING SYSTEM WORKED EVEN FOR A NON-DENTIST WHO NEVER SUTURED IN HIS LIFE (AND HE DID BETTER THAN SOME DENTISTS)!!

I just couldn't believe it worked so well, so fast!

So I decided to try it again....

on another group of dentists...

On a young dentist, old dentist, experienced and non-experienced…

on my assistant....

and as a self-study without my involvement.

BOOM!

It crushed yet again!! Almost any dentist going into this training with fear and uncertainty, comes out the other end with clarity and confidence.

It tested it in different venues on small groups and I kept hearing the following feedback and versions of it: “I finally get suturing!”

What I couldn't believe was that there was very little for me to do. Not that I’m lazy or don’t like to interact. You know I love teaching but this was totally different than anything I’ve done in education for the past 20 years.

What's so unique about it is that it's simple because most of it is

(a) INTUITIVE and

(b) broken into short concise tutorials

(c) You immediately implement what you see

(d) Step-by-step “Cookbook” recipe for each suturing technique.

It does NOT require:

• Years of practice
• Being a specialist
• Talent or special skills
• Understanding complex concepts
• Experience with surgery (although it helps in order to connect to the real world)
• Gross pig mandibles, bananas and oranges to suture.
• Many instruments and materials

 After testing this, I wondered,

"Could ANYONE learn this just with the resources I’m providing and without my direct help and physically being there?"

I mean, I couldn’t just disappear from the scene but I intentionally stepped back a little. I was slightly more passive in the teaching and “let the thing do its thing”. When asked for help, I was there to guide, encourage and support but constantly directed to the resources.

I wanted to see if this could work in a small setting because I had a great vision which I’ll share with you later.

Did the dentists with experience have an advantage? For some, sure. I had some advanced surgeons go through this and they were filling the gaps of knowledge they had.

But what if someone DID NOT HAVE that advantage... could they STILL master suturing?  

So I took that out of the equation.

I thought, this is so easy, let me just look at the completed zPADs and try to guess who is experienced and who IS A BEGINNER.

And so, I did. Some were great and masterful and some were “just” good.  I haven’t seen catastrophes or even mediocre performance. Everyone pulled it off at the end of the day.

I'm telling you, it worked again and AGAIN!!

It was such an enlightening moment for me as a teacher finding the path to helping the thousands of dentists struggling with suturing. A teaching system that doesn’t rely on me physically being there.

Until now around 100 dentists and 2 non-dentists went through the zPAD method and started suturing at a high level…

IN JUST 7 hours!!

And then, started implementing  with confidence in higher level surgical procedures. (except for the 2 non-dentists of course).  When you know exactly  what you are doing and feel comfortable closing surgical wounds, it’s a very calming and soothing feeling.

Pretty sweet “experiment”, right?

If anything, I think I'll learn a lot about how much people are willing to invest in their future.

Personally, if someone came to me 20 years ago and said,

• "You can learn all that you need to master suturing in 7 hours or 1 day…”
• …well, I would have said yes.
• If they said, "You don’t have to travel and have major expenses to learn this, just take it at home when convenient to you..."
• ... I'd be there – meaning at home learning this stuff.
• And if they said, “It's guaranteed,”
• It would be a no-brainer for me
