Sinus augmentation - TEACHING NOTES & TIPS

There is nothing worse then knowing half of the truth.

If you are planning to place implants in the posterior maxilla:

>95% of all cases you will have to deal with the proximity to the sinus.

So either you avoid placing implants in the posterior maxilla and lose on the opportunity to place hundreds of implants or you learn how to do it.

There are 2 main ways:

1. Osteotome (or indirect sinus augmentation)- lifting is done through the osteotomy

2. Window (or direct sinus augmentation) - lifting through the lateral wall of the sinus

Here's my #1 tip: Whenever possible - use option 1. Don't try to be a hero and show that you can. If all you need is 1-5mm of lift, and the membrane is thick enough, use this option. I go over this inside the Implant Blueprint Mentorship program. 

If you barely have any bone, even for implant stability, option 2 (window), is preferred. The key to success is not the membrane elevation, using sinus curettes or even suing piezo surgery.

The key to success in the direct sinus lift is the window outline.

More specifically, how far above the sinus floor you go. More on that inside the IBM.

Another tip is: Don't forget that the ridge is often destroyed too (due to the same reason that led to the extractions).

Often a ridge augmentation is needed on top of a sinus lift.

That means you need to build up the ridge + do a sinus lift.

I want you to understand the common sense and rational  of cases like that (very commonly seen in private practice, by the way).
